Customer Success Survey Request Form


This survey request form should be submitted by partners who are still in the Modern OPN* program and who want to qualify for incentive-eligible orders. Please refer to the Incentives Resources Center.

A successful implementation is determined by the customer and recorded via the Customer Success Survey, where a rating is calculated. Ratings reflect the customers' direct feedback on their overall implementation experience with the Partner 90 days post go-live.

*Food & Beverage (Restaurants) qualifies under Modern and Enhanced OPN until May 31, 2026
